How is possible to use an ultimate more than once within a few seconds? Is it gear or something else? I've had this happen many times now. If they can do it, I'd like to as well!

Dawnbreaker of Smiting has 2 effects. An up front hit of direct damage, and then 6 seconds worth of damage over time. The death recap treats the direct damage and DoT aspects separately. So that death recap is showing one activation of the ultimate. Even after the initial hit of Dawnbreaker, it will continue damaging you for 6 seconds.
Dawnbreaker only costs 125 ultimate, I hope this helps!

Pillager's Profit
(5 items) Casting an Ultimate ability while in combat grants 5% of Ultimate spent as Ultimate, up to a max of 20, to up to 11 group members within 12 meters every 2 seconds over 10 seconds. Group members can only be affected by this set every 45 seconds.
Major Heroism:
Grants 3 Ultimate every 1.5 seconds
Minor Heroism:
+1 Ultimate every 1.5 seconds
Arkasis's Genius
(5 items) Whenever you drink a potion while in combat, you and 3 group members gain 44 Ultimate. This effect can occur once every 30 seconds.
Bloodspawn
(2 items) When you take damage, you have a 6% chance to generate 13 Ultimate and increase your Physical and Spell Resistance by 3731 for 5 seconds. This effect can occur once every 5 seconds.
Dead-Water's Guile
(5 items) Whenever you kill an enemy, you gain 15 Ultimate.
Hide of the Werewolf
(5 items) When you take damage, you generate 6 Ultimate.
the first hit is initial hit, the second one you see is part of the dot. If you lived long enough for the dot to tic again you would have seen the recap have 2x by the second dawn breaker. There are other instances where someone can kill enough players to get their ultimate back (with the passives simon posted about ), but this case was the nature of the ult used and recaps not being formatted clearly

I meant that damage he/she hit you with was weak, others already pointed out it's not two separate ult drops seconds apart. Was this from a bg? Idk numbers there, don't play that content. In CP PvP I woulda just ignored that player or saved them last to kill, as they're not a threat, like pvp "tanks".
You must have taken a lot more damage from other players prior to what the death recap shows, & that templar came in when you were around half health, give or take, thinking a db ult dump would finish you...it was a wiff and resorted smashing jabs.
Hopefully we get a more detailed update to death recap someday.
Quite common when bombing. You only need about 10 kill blows to get another ultimate instantly. It's not that practical because you still need to wait for synergy/enchant/procs to cool down which is still ~20 seconds.
