It's not necessarily the fact that these things are in the same Categories. They could split them up, but it wouldn't do any good unless they made additional layers on the models. That's the actual reason you can't layer most things; they all go into the same layer. So Bosmer Antlers are on the same layer as beards, for example.
They really do need to add more layers to the models. Facial Hair, Makeup [Eyes], Makeup [Lips], Eyes [Special], Eyes [Covering], Facial Marking [Scars], Facial Marking [Tattoos], Body Marking [Scars], Body Marking [Tattoos], things like that to break up the model so we can actually customize our characters more.
Oh yeah, it's definitely possible. But simply splitting the various things into separate categories in and of itself won't do anything, they'd need to add those additional layers. They've done it once before.
Originally (and until some time after the crown store was added to the game) markings and adornments could only be chosen in the characrer creator, which only lets you pick 1 head marking, 1 body marking and 1 adornment per character. So for example my khajiit couldn't have ear rings and ear tufts because both are adornments.
It's only after they started selling markings and adornments in the crown store (incluing the packs with the character creator ones) that they split them into more categories. Now my khajiit can have both because the ear rings are an adornment and the tufts are facial hair, she could have another adornment too as long as it's one from the 'major adornments' category.
I'm not saying it wouldn't take some time and effort for someone to add new layers and sort everything, but it is possible to add new ones.
(Also choosing an eye patch as an adornment used to give the character a blind eye as well, which persisted even in situations where the eye patch was hidden, like when wearing a helmet which hides adornments to prevent clipping but keeps the eyes visible. A few years ago they changed that so the eye under the patch looks normal and the blind eye, so they can edit and seperate existing cosmetics too.)

I have that one, can't check the name because of maintenance but it's in Facial Hair. I don't remember it coming back either!
Edit: It's account wide so any of your characters should be able to use it unless you have another account
