When using a gamepad on PC since the last update I'm finding that very often, if I do a medium attack with a staff (lightning and resto are the most obvious) then even when I completely release the trigger the attack continues until it becomes a full heavy attack. So I'm not just tapping the trigger to perform a light attack, I'm holding it down for a half second / second and then releasing the trigger. Despite releasing the trigger though the attack continues, even if I turn and face a different direction, until it becomes a full heavy attack and naturally ends.
