I love him. But I wouldn't necessarily want him as a companion either... not as a free-floating skull and not attached to any other body.1 -

The floating skull? That's Morte, first companion and old confidant of the amnesiac protagonist in Planescape Torment, immediately thought of him because his personality is quite similar to that of the Augur of the Obscure. Annah is a Tiefling, and another companion. When looking for Morte images I found that painting and just loved her annoyed expression; perfectly fitting for both skull-entities.
As for *what* he is... (spoiler for a plot twist in a 21 years old game)A tormented soul ripped from the Pillar of Skulls on the first layer of the Nine Hells, where the souls of those end up, whose lies cost another's life.1 -
