I got the last fish in Cyrodiil, received the Master Fisher achievement, checked and confirmed, even linked it in guild chat and got accolades.
Then, after I rode a ways toward a keep, I lagged and dropped to character select.
Upon logging back in, I no longer had the achievements.

✭✭It wasn't lag, pretty much the whole zone on Blackreach crashed to char select and got what appeared to be a 10-20 minute rollback. We had quests we already completed and turned in back to half done, etc... was a hot mess.3
