How this set work? Legacy of Karth

Says enemies take disease dmg while affected by my alchemic poisons. Does that mean I have to have this set active on both bars? Like if I only have on from bar and my poisons proc and the moment I swap they stop taking disease dmg?
Also can I have 1 type of poison on front bar and a different type on back bar can they both be active at same time?

Unless someone else has already tested it, this should be relatively easy to check, go to a target dummy, attack until your alchemic poison procs, check the tick damage, then swap bar (to the one where the set is not active) and see if your tick numbers change. Some sets carry over to the bar they are not active on, some do not.

Once an alchemic poison procs on an enemy, it stays on the enemy for the duration, regardless of if you swap bars. Therefore no, you do not need to have the poison active on both bars.

Yes, you can have two different poisons on, one for each bar.

This set seems mega under-tuned given it's crazily niche proc condition.

Poisons last, what, 4-5 seconds for most effects, so you are getting 5 ticks of this proc and then another 5 seconds of enforced downtime while you wait for your poison cooldown to end.

Sure, you could use double poisons but if the poisons overlap it's not like you're getting double the proc ticks, you're simply using two poisons for basically the same damage.

But even if you could perfectly time your poison procs and had 100% uptime on the Karth proc you would still have a single-target only proc DoT that does less per tick damage than tons of the other popular proc sets, many of which are also AoE!

This set would be spicy if they combined the effect from Assassin's Guile and Karth into one set (e.g. increases durations of alchemical poisons by 4 seconds and adds the proc DoT) but as it stands, this set is simply way too weak to justify the build contortions required to make it work.
