Labyrinthian (Blackmaw) - Boss disappears upon attacking

I've seen this more then once, where the Blackmaw boss in Labyrinthian would just disappear when attacking. It usually happened directly after it spawned and never after he killed those Skeletons that were holding him back. So when you use an ability while he is still chained there is a chance, that this will happen. However it did never happen twice in a row. I was able to re-create it a few times, but inconsistently.
The current version i believe is 6.0.6. The servers are currently down for maintenance, so it might be fixed after the patch but i'm not sure about it since i could not find an existing bug report about it.
