How many copies of a Mythic item can we have? And what does the # codex mean?

If we wanted to collect mythics for each or our characters, is there a limit to how many?
Also, what does codex mean? Each mythic has 15 codex, so it might suggest we can only collect three?

The codex entries are small bits of lore.
Some have more than 1 lore entry.
