Dungeon Finder Glitching

Prior to today's maintenance on occasion and even now after maintenance I am seeing this set of glitches.
After queuing at random I will receive a Ready Check that has 00 Time remaining that can neither be confirmed or declined. I cannot leave queue either after this occurs to re-queue because the Leave Queue option is greyed out as the queued time continues to tick. Logging out is the only way to reset.
After queuing if I change zones I will receive a message that I or someone from the party declined the invitation and the queue locks up with no option to leave the queue and the entire menu listing for queuing is greyed out.
