Without going into too much detail, the survey touched on a couple dozen topics; first quantifying the level of importance to the player, and then ascertaining the player's satisfaction with the implementation or trend. There were also a few crude comparisons of Elswyr to past expansions. With one exception (below), I'm not going to go into what was asked.
The mere existence of the survey brought me hope, as did the existence of two of the questions. What saddened me was the total absence of whole topics I know many people value (not necessarily me). Even if companies have no intention of ever reading the responses to certain hot-button topics, merely including such questions at least acknowledges the topic and gives people hope that the company is gauging interest beyond the few squeaky wheels on the forums. When they don't even take the zero cost step of faking interest, I have less hope than I did before the survey showed up. They just seem oblivious.
There was one question that boiled down to "Do you like having a great time?" The answer is the same for all sentient life in the universe, and the only reason to include such a question is to gauge how many respondents are willing to take a survey just to click random answers. Questions like that make me feel more like a lab rat than a valued customer. Perhaps they're surreptitiously gauging my interest in Skeevers as a playable race.
