I think it's about the maelstrom motifs. Only my characters that use maelstrom style on backbar experience this, sometimes my frontbar weapons disappear from my hand completely and never come back unless I re-equip them.

Try looking under your feet. No, for real. Also this should get fixed in U24.1 -

Nope, it's common, and not restricted to any one style/motif.
Just go into collections and toggle headwear on/off.0 -

BtW, it seems to simply fix itself when using a wayshrine as well. ...so, if it happens, just see it as a sign the gods tell you to take a little break from all that adventuring, pop into a city and unload your loot, go get a snack of a bio break, whatever, then carry on questing!

Skullderic✭✭✭If you open you're (U) Collections click on Outfit Styles tab & Exit it, that will get it right most of the time.
I have a few Chars it does it on a lot, if I port or WS to new zones I have to make sure its on the front (#1) bar and it don't do it.
