Companions, working as intended or bugged?

I have run many quests now where you get a companion to help you during your quest. The unfortunate part is most of these companions rarely do anything and it seems the quests are tuned for you to actually receive help from them. is this how they are suppose to act or are they bugged?

    Many of the npc's do little to no damage. Some are there to follow you through whatever cringing when ever a fight occurs some at least look like they are supposed to be fighting.

    If you were actually counting on them to be really helpful though ....not going to happen. You are the hero....if you are not on a solo only quest and need help I suggest bringing a player to be your buddy.

    Solo only well I wish you luck then cause really most of the npc's are hitting things with a plastic sword.

    I've seen some that can put out some damage. Even those ones seem to bug out though, and just stand next to the enemy doing nothing after a time. I'm thinking they may have a damage cap or something, that once reached, they shut down and stop attacking.

    If they are injured badly they will stop doing anything.

    Oh, I don't mean damage taken. I mean damage put out.
