You said you move without penalty when sneaking, even without gear - are you a vampire? Feed and Blade of Woe share the same button (synergy). Try moving closer to your target maybe, if that's the case?

been there, I WISH I had any ideas how to fix it. but restarting the game a few times until BoW reappears... temporarily seems to be the only thing that works. some of the time. its not an addon issue. I have multiple characters that steal and murder and only one of them got hit by this bug. disabling addons didn't help anyways. its also not vampire/werewolf issue, because my bugged character is akhajit were, but so is my NOT bugged character. why does one work just fine and the other - loses ability to BoW? no idea. it is apparently a bug that's been in game for a number of years, but it appears so randomly, I'm guessing that they still haven't figured out what's causing it.
I'm sorry OP that you are dealing with it, all I can offer really is solidarity and wishes of luck. try restarting a few times more, see if the golld old fashioned turn it off and back on again decides to randomly fix things.
What level is your character?
I found out a while ago when BoW suddenly stopped working consistently for me that I was only having trouble with it on my characters that were under level 10. It started working properly again once I hit level 10 with each of those characters.
(I was having the same issue with 'Feed' on my sub level 10 vampires.)
So now I don't do the initial DB Blade of Woe quest ('Kill an innocent in Anvil') until I reach level 10 with a character.
(Same thing with the vampire quest.)
I haven't had any problem with BoW or Feed since.
