Elsweyr mural wall bug

My mural wall bugged while trying to place fragments. I had picked up Orcrest fragment and Verkarth fragment from Predator Mesa. Once at wall I could not place either and caused a strange log out crash. I could place Orcrest once back in game but the Verkarth fragment is gone, not on wall, in inventory, or delve where found.
The achieve lists it as collected but it is not on the mural wall in Riverhold.

Sorry you're having this issue too, it worried me for a few days because I was short a fragment section. I picked up and placed them one at a time to not repeat losing another.
