Performance degradation

First of all I know, I am sure this is just me so I thought I would come to you guys for help.
Ever since one of the more recent patches, the performance in my game seems to have tanked. I have had to disable most of the graphical options which pains me, in order to achieve a consistent frame rate, but even then I could be roaming through the world and I will just get random stutters, where my FPS will tank from 100fps - 50 fps then back up to 100. I don't remember this ever being a problem before, but I am sure it's just my machine because I don't see anybody else mentioning this.
My specs are as follows:
Ryzen 1700 4Ghz
1080TI OC
16 Gb DDR4 3200Mhz
G Sync 100Hz 1440-1440p monitor.
Now I have done most of the little tweaks in the ini file to try and solve my issue, and I have watched many videos. I have had to turn down most settings to low to avoid as many stutters as I was getting, and to get above 60fps in most towns. But like I said even then I get random stutters everywhere, town or not and it just knackers the experience.
Anyone have any ideas?

Most of my problems occur when I get wonky connection to the server with it being most noticeable in PVP where seconds matter. What things have you disabled and what do you keep on?0 -

These are settings I have disabled. It's rather annoying that I have to disable and turn down most settings to get a playable experience. Even back in 2009 when this game was in development, why did they have to program the game to only run on a single core? It blows my mind... then again it could be my computer, I dunno.0
