The only time that a piece of outfit can disappear is when a different type of weapon is switched

✭Sounds like a bug because I've stripped my character's completely nekkid and they still have their outfit on. Only time anything changed was when I equipped a different weapon type.2 -

✭✭✭✭✭I change sets often and do not lose the outfit. It does get covered up if a costume is equipped.
If you are on PC and are using an addon to change your gear it may have a setting to change to a saved appearance.
