Pets aren't supposed to die from any and all damage, but there are several specific encounters in while pets still die:
Mazz totem will kill pets *after* it dies/when boss cages
Fang lair caluurion will kills pets based on totem/enrage at 25%
I haven't ran CoH2 myself, but it very well could have died there too. He didn't list a percent, but it might have been when phase changed perhaps, as I noticed they ususally die to encounter phase shifts <totem deaths/cage/enrage>.
What games do you play where "all" does not include everything?

Combat pets will now take 0 damage while in a Dungeon, Trial, or Group Arena.

This is irrelevant since it only affects outside of group dungeon and trials. It was to fix where pets were dying outside of group instances.
The relevant Patch Note comers from v4.2.5Combat pets will now take 0 damage while in a Dungeon, Trial, or Group Arena.
