Question about Glyphs

When I apply a glyph to a weapon, let's say a glyph that adds a damage shield for 5s, how can I trigger that effect in combat? Do I have to do LA / HA or do weapon skills also trigger it? What is the chance of triggering and is there a cooldown?

Another question: I noticed that armor glyphs recharge with repair. Is this true of weapon glyphs also or do they need to be replaced after they get depleted?

    Triggering effect in combat: hit them with the weapon (light attack, heavy attack, weapon skill)
    Chance of triggering: 100%
    Cool-down: yes (length varies from glyph type to glyph type, infused trait on weapons reduces cooldown)
    Charge: the meter that appears on enchanted weapons is not durability, but the enchantment's charge level, use soul gems to recharge said weapon when the charge is low
