I think it's supposed to be statue of Malacath's head.

Thanks. I remember a quest in Orsinium that this head was a door into a dungeon, etc.

I thought it was Malacath also...

Thats definitely Malacath. Theres two features about Malacath that artists often depict when creating sculptures and carvings of him. Hes often depicted in a fit of rage and with two sets of horns on his head. Both of which are shown here in this head statue.

There's a house in Orsinium with a shrine to Malacath and this head is in there too.
