Encountered a new bug yesterday night.
Location: City of Ash II, Veteran Difficulty
Issue 1: The giant golems (blanking on the actual mob name - The ones such as you encounter while working your way towards the ritual interruption) would cause instant death to random group members during combat. There would be no warning or visible attack associated with the death. Sometimes it would say I was hit for 1-2k damage, but that is certainly not enough to kill me, other times there would be nothing in the combat log. Once, it instantly killed the entire group after being hit by the first attack, and we were spread out over a large area, so I doubt it was a critical hit AOE.
Issue 2: Group members (it happened to every single one of us multiple times) would get stuck in the revive screen. We could move the camera around, enter the menu, or do just about anything except press A or X to revive. The only fix was to re-log. There did not seem to be any correlation that we could find so far as manner or timing of death.
Honestly if you have to pick only one issue to work on, please work on #2. Dying for no reason is frustrating, but not being able to revive reliably made a 40 min dungeon take 3 hours...
