I only had to do the Privacy Policy once.

I have to accept it every single time I run the client. It's annoying.

Yeah, for me it was once. Not once per account, just once. That implies that the acceptance is stored locally. If something goofy is happening locally that prevents this information from being stored and retrieved, or is being cleared, I could see this happening.

Found the solution..............if your user setting in Documents, ESO, Live is set to read only, it will prevent it from updating the policy version. I actually just changed the number on mine from 2 to 3 and saved it and it was fixed when I went in game. If you do not want to go thru the trouble of looking for it in the settings, just set the file to normal in properties and go in game and accept the policy change, exit and then make it read only again and that should fix it.
