Any list for hard mode requiremets?

I can't find any, not even individually for each dungeon and the quest journal doesn't say anything about what needs to be done either. I think the journal tells you at some point while in the dungeon but having to read up the requirements mid-dungeon is rather inconvenient. Not to mention some quests still speak of "gold keys" which aren't even in the game anymore.

Hard mode is one of two things:

1: read the scroll before killing the final boss.

2: the actual undaunted quest will instruct you on what to do under "optional steps"

Option 1 is the easiest because all you gotta do is read the scroll and burn the boss however you like. The 2nd option can be tricky because you need to follow steps, like in Banished Cells 2 you need to kill the boss with 3 daedroths alive. So you actually gotta pay attention to what's going on

Yes but these optional steps should be available in the quest journal from the start not during the dungeon. For example all these "keep x alive" sort of tasks are barely worth doing since nearly everything I have is AoE. There is no point for me to queue up veteran if it's not clear what I'm supposed to do and it's likely going to be messed up anyway. A list or updated journal would help with that.

In the case of Banished Cells 2 the tank needs to take the 3 daedroths away from the boss so they don't get caught in the AoEs. As far as the journal just make it your active quest and you'll see it on the left hand side of your screen.

Kill Rilis with three Daedroth still alive

Darkshade Caverns II
Defeat the Engine Guardian without pulling any of the levers

Fungal Grotto II
Defeat Vila Theran without stepping under the husk

Spindleclutch II
Defeat Vorenor Winterbourne without killing any of the innocents

Wayrest Sewers II
Kill 15 zombies during the fight

Crypt of Hearts II
Have 4 wraiths alive when Nerien'eth takes the blade

All the other ones
Defeat the boss after reading/destroying/defiling/whatevering the book/statue/tome/scroll/boss's mom/etc.

The only true HM's are the dlc's and tempest in my opinion...
