If you are using the Master Merchant 1.9.5 add-on, then it is probably interfering with the E-Mail User Interface (UI). On one occasion, when I opened the E-Mail UI, the first message was one with an item attached, but neither the content of the message, nor the attachment, were displayed. The game client did not display a UI Error message. However, after I disabled Master Merchant and reloaded the UI, the message was displayed correctly.

Also, if you are using the Awesome Guild Store add-on, be sure to keep it updated, because it interacts with Master Merchant as well as substantially changing the Guild Store/Guild Trader UI. Since the most recent AGS update, I haven't had any problems that I have been able to identify as being caused by AGS.

If you are not using either of those two add-ons, then consider whether you use any that change the Inventory UI (bag contents) feature. Since E-Mail attachments are typically transferred to the character's bags (and/or currency), any add-on which changes or interacts with the Inventory UI in any context has the potential to interfere with the E-Mail UI.
