Xbox One - NA acitivity finder continues to group DPS and no tank and/or healer (Dec 16- Feb 2017)

This has been an ongoing issue since December 2016 but possibly longer.
The activity finder puts us in a group but it's not correct and typically it's existing groups but in this video I'm being added to an existing group
It results in ppl being voted out over and over again and having a 15 penalty
