Questions, for clarity on physical editions of ESO: Morrowind

1) Do physical upgrade copies come with a disc for the expansion, a code, a disc AND a code? (it's not specifically mentioned on the pre-order page or Amazon, doesn't even make mention of a code.)
2) As above, does the physical full edition work in any of the ways described above but including the base game?
3) Is said disc / disc-set a complete install of the game up until the Morrowind patch? As in, could one install the entire game off of this physical edition on that patch?
For the Collector's Edition, specifically:
4) What kind of dimensions/material can we expect of the map itself? If not specifically, is it more something one would hang on their wall or a literal map one would set on their desk while they play?
5) Of what material is the Dwarven collosus statue going to be made, pretty much the thing I'm least interested in but I'd like to know.
