Poisonous Serpent - How does it work exactly?

Looking at some sets and came across this, I'm building a Stam DK for PvP and just wondered if anyone could clarify on how this actually works?
(2 items) Adds 967 Max Stamina
(3 items) Adds 129 Weapon Damage
(4 items) Adds 967 Max Stamina
(5 items) When you deal damage with a Light or Heavy Attack against an enemy who has a Poison Damage ability on them, you have a 25% chance to deal an additional 6800 Poison Damage.
Does this basically mean that if I've got poisonous claw or injection on them light and heavy attacks can proc the effect?

It wasnt me, i just played with the proc chance and from what i can tell, it doesnt have a cooldown at allMechanically Challenged, PCNA competitive raid guild
