It's located near the exit and is across the other side of a broken bridge, yet I've been running around like a madman trying to find some way of getting it. I can't seem to get it from the outside (dropping into the fort via a cliff, nor have i got a hope in hell of making that jump. I just keep sliding off the rocks next to it. Any help would be appreciated.

Enjoy the game, life is what you really want to be worried about.

I assume he means the "Proper-Life: Three Chants" one. If you exit the East door from Fort Grimwatch and turn immediately to your right you should be able to jump up and through the small gap in the wall and drop down onto the ledge with the lorebook (I just tried it now). Alternatively, the same lore book can be found just SouthWest from the Fort Grimwatch wayshrine or directly West from the Old Town Cavern crafting site.

If you're on PC I would recommend the Lorebooks add-on which is great help for finding and reaching lore books.

I tried that and couldn't make it through. Obviously didn't try hard enough. Will attempt it again or just pick up the other one you mentioned. Thanks!

I just found this post while trying to remember (and failing to do so) how to get this lorebook. You'll need a mount to get it (at least it makes getting it easier). Go out the north gate of the fort and along the wall East/southeast of the door are 2 breaks in the wall. One you can jump through easily and a second one farther from the door with some sand-colored rocks below it. Get on your mount, face that second gap and rocks, then take a running jump towards the rocks to get on top of them and reach the gap. It might take a few tries, but once you do, dismount and drop down the other side to the ledge where the lorebook can be reached.
