PS4 entire trials groups being logged out. NA

Twice last night while in Vet Hel Ra my entire group was logged out and placed in areas all around the game when we logged back in. Once while fighting the second set of bosses and again while fighting the final boss.
Most of us were placed back where we were before we ported in to the trial but some were sent to random locations.
Is this wide spread? Already known and being fixed or a completely new issue? Never had it happen before this little event started. Besides this the lag has been terrible as well.

wolfxspice✭✭✭can confirm xbox, progress is also being lost when this happens i lost motif pages, and a friend lost cp. I'v heard this is happening in every single trial accept maelstrom.I'm a casual now0
