Abandoning a Sacrament, after a partial fail, does not reset the quest objectives like it did with the TG Heists.
Example: If you abandon the Sacrament with 2/5 detects, when you reacquire the quest, you will still be at 2/5 detects.
I mention this because you said there are times when you instantly see the Overseer. This is likely to be the cause.

Actually - if you have alerts just abandon the quest, leave the zone and go kill a few things somewhere else, go back and start the quest over. Your alerts should be gone

✭Okay... so I MIGHT have found a workaround for anyone suffering with this... it appears, that if you swap weapons before making the kill - you could get credit for it. So far its a 100% success rate - but I'll report back if I get any failings.
Sadly if you aren't a level 15 yet - this won't apply to you.0
