"Litany of Blood" Help!

I'm trying to figure out how to start "Litany of Blood." I have done some research url: http://elderscrollsonline.wiki.fextralife.com/Litany+of+Blood and it says you must complete your first Sacrament Quest to unlock the Litany of Blood, but I have completed my Seventh Sacrament under Flawless Sacrament and I have not received any form of options for Litany of Blood, also I tried talking to Nevusa the mysterious Dark Elf assassin and no luck. What must I complete in-order to unlock the quest "Litany of Blood?"

you spoke to her, you read the book, you completed the quest & received the book as a reward. so you have completed stage one of the quest
now you must make your way through the list & kill all the people across tamriel, the list can be found in your achievments, and there are some great guides on youtube
once you have killed all the targets (using blade of woe) pop to the sanctuary and speak to the woman again, she will reward you with your polymorph0
