COMPLAINT: Achievement Unlocked!!! NO ACTUAL ACHIEVEMENT GIVEN

In-game message pops up saying achievement unlocked, don't get gamerscore or actual achievement. The most recent occurrence of this was about 5 minutes ago where it said "Achievement Unlocked: Slaughtered". I am very disappointed about this as I very much care about my achievements and I'm sure I'm not the only one. Thank you for taking the time to read this
