Blade of Woe not working?

I've been trying to use the blade of woe on ps4. I press the right buttons, hear a sound effect, get a strange glitch animation and the people remain alive. Am I doing something wrong?

I've had this happen on PC a couple of times either when there was something blocking me a little bit, or when the target moved out of range as I was hitting the key. If you're unobstructed and the target isn't moving away (or if you're close enough that the target doesn't move far enough to get out of blade of woe range) and this is happening then that definitely sounds like a bug.

Are you a vampire? Someone else complained that since the feed mechanism is the same they were trying to feed on someone instead of use the blade. They said adjust your distance maybe if that is the case. Otherwise idk.

do you have in the settings-gameplay, toggled, "prevent killing innocents" turned off.

I've had this happen when I try to attack a target that is within a group... I hear the sound, the screen flutters like it's going to the Blade of Woe kill animation... but then just abruptly stops... I attribute this to being 'seen' by someone in the group at the same time that I activate the BoW. I've also had it do extremely funky animations when killing vampires... the animation starts but then immediately the vampire falls to the ground dead while the blade animation continues stabbing the empty air.
