JinMo92Soul ShrivenI had the same problem and did Maelstrom without dying with a 15/15 vitality bonus with a score too. I even recorded a video of me beating veteran maelstrom arena with a score without dying and the achievement or title was nowhere to be found after completion. I posted a thread about this too and submitted a ticket to ZoS as well. ZoS response was "we will escalate it to one of one of our technical teams" and they never got back to me. I hope you get your title and better luck than I did.1

✭I can't imagine how frustrating this would be. Keep pushing to get what you deserve. If it makes you feel any better, if you did it once, you can do it again. But just because you can doesn't mean that you should because you should have gotten it the first time. If I was in your position, I would be a sad man indeed.

•Perfect Run: This achievement can no longer be completed from Maelstrom Arena runs that were continued from a saved quest."
