Your game files are corrupted.
No idea about xbox but on the PC we have an option to repair the installation. That usually fixes the problem.

It's Orsinium only....then I can confirm that my resolve was the hard drive. So if you're trying it on both hard drives, make sure to delete it from one and only run it on the other. That seemed to resolve my issues.
The only other idea is to factory reset your Xbox one.
