It has always been hard to get a vampire/werewolf bite "naturally", i.e. by getting bitten by an NPC rather than a player. This is because they only spawn at night (in-game night, that is) and at specific locations at each faction's fifth zone: Rift, Reaper's March and Bangkorai. They also don't spawn every night.
In addition, the NPCs can get killed, and if they do, they will not respawn again. On the PC, some players would purposefully kill every werewolf/bloodfiend in order to sell the bite themselves. It can also happen that someone kills them unknowingly.
This thread contains a map of spawn locations in all 3 zones mentioned. Try looking there during in-game night. All you need to do is make them attack you and block/heal until you see you've got the passive for lycanthropy/vampirism, which means you're infected. You can then just leave the NPCs there for others to get bitten.
