I've had the problem at least since the Orsinium patch, I started noticing it when i turned on the subtitles which identified it as the "Soul of Dusk". It happens everywhere.
The NPC has Molag Bal's voice actor or someone very much like him
lines include "you consider yourself a benefactor of Daedra, mortals?" and "Lyraneth deserved everything we did to her"

Logging in and out did not help, this issue has been going on for a long time.

Most in-game UI issues (including NPC, quest problems and missing crowns/entitlements) can be resolved by logging out for five minutes then back in which will load you into a new game instance.
