Weapon swapping with max attribute bonus

So I'm thinking about getting a two piece endurance set for my tank that will consist of a neck and two shields. Will I temporarily lose the bonus to health when I swap? I don't know how weapon swapping works mechanically, do you un-equip then instantly equip weapons? I don't want to basically deal myself 1500 damage when I switch weapons.
