Upon achieving the achievements for quest completion for an example, complete 40 quests in malabol for. Once all forty are completed , are there still quests to do or is the zone all emptied out of quests?

There's still more, the achievement only requires you to do almost all of them. I'm not sure why it's not actually all, and the amount that's left still varies between zones, in some cases it could be a mere few, but others, almost a dozen.

If you're looking to truly complete all of them, since there's no quest list, I'd advise looking for offsite lists. You can also look at your map of the zone (if it's complete) and seeing if there's any black quest-area icons left (ruins, villages, trees, etc, all the quest-hub icons)

If you're on PC, I think there was some addon that did pretty well at showing locations for quest starts, although I can't remember the name of it.

I find that in most cases there are usually a few side quests left in the zone once I've gotten this achievement. I use uesp wiki to check all the quests in each zone.

    In your case: uesp.net/wiki/Online:Malabal_Tor#Quests

Back at launch, they actually did require you to do all of the quests in a zone. Including little, "Oh, thank you for completing my quest, but you skipped a step, you need to go back to that other area, and do their thing" meet and greets, which won't actually pop if you've already done the previous area.

It made some zones like Shadowfen nearly impossible, because you'd actually need to do some of the quest nodes out of order... and I think in the case of Shadowfen or Stonefalls, you needed to do two mutually exclusive quests, so the achievement was impossible to get.

The head count for quests in a zone should be ~the number of quests you need to flip all of the map white, now. I might be a little off there, but not by much. If you pick up meet and greets, that will send you someplace else, or steel some whiskey for a dude, then those will come off your tab. So you can usually get the achievement with a couple points on the map undiscovered. Or, rather, the achievements track the number of unmissable quests. Quests that you could have missed on the way through, or gotten locked out of, or don't really contribute to completing the game (like the frog race) can be safely skipped now.
