    I guess the "bug" that sometimes an item upgrade with 100% upgrading mats would still fail.

    I made hundreds of purple and golden crafted items for myself and guildies and 100% chance of upgrading gave 100% upgrading. So I guess that's been fixed.

    So that would be an Improvement Bug then.

    And, yes, if you have a 100% chance to succeed, you will succeed. Have a 90% chance to succeed, you will fail 100% of the time.
