An issue with the Assassination ability - Ambush (Teleport Strike)

I looked around but didn't see any other threads on this, though I'm probably blind and missed them

I have an occasional issue with this ability, whereon usage it'll show me doing the "jump" before I actually teleport. However, I don't teleport and my weapons disappear. I'm stuck like this for about 30 seconds and then it moves me back to where I initially used the abilities with weapons in hand.

When I use the ability, I can move around, but I can't attack or do anything and enemies can still attack me if they are attacking me where I was standing when I used the ability.

Do you notice a trend when you do it? Are enemies on a different height than you (jumping up or down to them)? I've noticed it before on PC when an enemy would just walk over the crest of a hill as I teleported and sometimes could get a little strange. When that happens, try switching weapons. I've noticed that works for the DragonKnight wings glitch.

I have the exact same problem. I have to run around trying not to die until my weapons reappear.

Having the same exact issue on PS4. There's no pattern to it other than it just randomly happens when I use Ambush. Weapons disappear, I lose all functionality beyond basic movement, and I run around hoping I don't die over the next 30-40 seconds. This only happens with this particular ability and I've not had it happen with anything else.
