A while back they fixed something so you could not use this to pull down enemy players from Keep Walls. Don't know how this affects PvE as I used to use this to pull mobs up to me or down to me.
I stopped using Chains after my DK got into the 20's. But I do remember that I also had to have LOS between my character and the mob (not the camera and the Mob).

It is a weird bug but one solution to it is if you do a heavy attack on a random enemy it should fix it for a temporary amount of time. It usually happens in the same spots for me every time.
This happens to me in Cyrodill the 90% of the times!!!! always the same message! this bug was so frequently that I get so tired that I had to remove the skill complete because was almost not working at all!
Not only that! also I found another bug, when I use extended chains and "finally works" (HURRAYYYYYY!!!) sometimes I'm not being pulled to the target or the target is not being pulled to me! the chains just dissappear doing nothing! this was for me like the complete not go anymore.
If the player you target does a dodgeroll or uses a skill / armour set that provides a dodge chance it can happen that the chains are dodgend and nothing happens.

mmmmm ok... but this game has so many bugs, that how can I know that really has dodge change or is not a bug?
If you have a addon who say "dodged", it's dodged. If you don't have such addon… you have to see if he dodgeroll just before, or if he is transparent (medium armor dodge skill) or if he is in a red bubble (NB's dodge skill), but it's pretty difficult sometimes. Ah, and if during the chain animation the enemy is out of range, the chain do't grap him either.
