Werewolf not changing

I've just gotten my third Werewolf, but for reasons beyond me, when I go to change into my wolf form, my character screams, black smoke surrounds her but she stays as she was. The Werewolf action bar does appear, but it's just not the same without actually looking like a Werewolf. I've tried changing while not in combat too, and the same thing happens. My other 2 Werewolves are fine though.

The character this is occurring on is a Redguard Dragonknight.

Through some experimenting with the ww stuff lately, found out the costume bug is back again, but only for certain ones. Basically skeleton poloymoph and so on are fine, things like jester outfit and wedding dress block the avatar change again, along with a couple of disguises.

Old bug that has resurface since last patch, be sure to report it ingame.

Also can randomly happen without wearing such things, always has been around. :(

Same bug with the Rogue costumes from crown store. Reported it.

"I am a nightblade. Blending the disciplines of the stealthy agent and subtle wizard, I move unseen and undetected, foil locks and traps, and teleport to safety when threatened, or strike like a viper from ambush. The College of Illusion hides me and fuddles or pacifies my opponents. The College of Mysticism detects my object, reflects and dispels enemy spells, and makes good my escape. The key to a nightblade's success is avoidance, by spell or by stealth; with these skills, all things are possible."
