You typically get there after completing a quest in Shadowfen - you get shipped up there by boat. It is also possible to walk there, by going north past the fort at the north of Stonefalls into The Rift, and then going north through The Rift. Or you can teleport to someone on your friends list, or in a group with you, or in one of your guilds who is in Eastmarch.
