From my experience pretty much all MMOs charge you for something like that. Some might allow you to do minor modifications for free or for ingame money (like Eve Online), but you'll have to pay for complete/full edits in pretty much all of them.

Everyday hair/makeup/adornment/haircolor changes add gold cost to the total, "remake" shape/voice/size/skin/eyecolor changes add crown cost to the total, fiddle until satisfied, then click "purchase" and pay the combined price for your new appereance.
Possibly the easiest way to implement the whole thing... at least from our side.
That said, I hope there WILL be barbershop-specific hairstyles/tatoos/adornments you couldn't get at character generation, but have to unlock through in-game archievements or research or finding a style book... just like dyes...
Thieves guild is certainly coming methinks, the justice system paves the way for it... might be DLC of course. Dark Brotherhood, same.
Spellweaving... that one I guess is coming, someday, though I would think it also may require a good deal more work balancing then just adding a new guild with their storyline... still, maybe they'll surprise us with it coming sooner then I fear?
BtW, the only MMO I ever played that had full character remake for in-game money was Star Trek Online. Which made their profit through "Pay to Win", requiring all good stuff to be paid for through the cash shop (and the best stuff in lockboxes for even more money grabbing... as long as we never see lockboxes like that in ESO, I will happily sacrifice a sweetroll to the gods of zenimax every friday the 13th...)
