Even the best players of the game die in PvP, so that would not work for them.
Playing DiD is a noble goal, but not realistic in a game like this where you can be one shotted and aggro too many mobs by mistake. True, you can meta game and figure out what is going to happen to you before it happens, but that kind of takes the fun out of it.
That being said, if you have a high level crafting alt, then a low level character may never die if they stay on level with the mobs that they face and avoid world bosses and public dungeons unless there is a group around. But, is that really a DiD play through? That would be a discussion for another thread and probably could not be tracked by the game unless they made all crafted items Bind on Creation and that would go up like a lead balloon.

Other mmo's have this however ^^. Like I mentioned GW 1 and Lotro do.

Today I was talking in my guild about characters and dying. And suddenly I got to thinking; why doesn't eso have an ironman achievement yet? Ironman is a mode in many games were you cannot 'save scum' so you cannot go back once you have lost/died and must start a new. Many other mmo's do it aswell like guild wars 1 & lotro.
I would like to see something like this aswell for Eso, 5 achievements to level a character to 50 without dying a single time and every 10 levels you would get part of a reward (so those who cannot make it to 50 still get a fair share of rewards). The rewards would be in the same style of the 'undying', basicly people who do not die. Every rank more rewards would unlock and they will get progessively more awesome while not giving you more power, it's mostly for show-off purposes.
Rewards
Level 10 :
<The unblooded>
Level 20 :
Cosmetic armor set, cool name would be 'of the undying'
Level 30 :
One silver dye
One gold dye
Thematic pet fitting the 'undying' theme
Level 40 :
'Banner of the undying' + <of the undying> title
Level 50 :
<grandmaster of the undying> title
Grandmasters regalia cosmetic set
Grandmasters servant pet.
These rewards would unlock depending on the level you get without dying once you die you cannot get the achievements anymore but you can continue to play with your character.
Rules
Last but not least some rules should be put in place to make sure that the achievements are truly 'won' and not grinded with friends.
For level 10 you will need to have completed the corresponding fighter's guild/mages guild and main quest lines
For level 20, same rules
For level 30, same rules
For level 40, same rules
For level 50, same rules
This will make sure that anyone who is doing the achievements has shown that he cannot die in 'private' situations aswell.
Conclusion
Overall I think that these five simple achievements + rewards could add many hours of enjoyment to eso by adding something that is 1.hard to achieve 2.soloable and 3. it will give an extra edge to leveling new characters

The point of permadeath is to make your decisions final, so you don't make them lightly - and you are more emotionally invested because you know you can lose your character forever. It's thrilling and a little scary, and so very sad when you finally let your character go.
