Quests not updating properly Fighters Guild and Reapers March Willowspring Cavern

I carry the title of Fighters Guild Victor, carry the Prismatic sword and no quests are there in "Guild" category.
Problem1: No portals available to Earth Forge except the original portal to get there. cant find any dialogue options that I have missed.
Completed the quest for Willowspring town/cavern Mage and Spouse etc
my map shows it all white.
All the atronarchs are green haloed and not aggressive.
Problem2: The Reapers March explorers striking locales achievement will not unlock because it still shows Willowspring Cavern as not yet discovered.
This is also holdiing up the overall AD achievement.

Really? Others have reported either deliberately or accidentally deconstructing their Prismatic Weapon... You can also use it for research.
