Several Quests Just Disappeared From My Log

So I log in and I have around 6 or 7 Auridon quests. I start on one, Eye of the Ancients.
I kill the Captain and get no drop, so I go get the other two stones. I come back and kill the Captain again, still no drop.
I log out and back in .. and the quest has gone, along with 4 others including the zones main story quest chain.
When I got back to the quest giver I get to start it all over again. I have no idea what quests I've lost.
Another patch another game-breaking problem.

✭I took the quest again, killed the Captain again, still no quest item.0 -

✭I log out and back in and the quest has reverted to the previous state, ie. talk to the NPC who sends me off to kill the Capatian and get the other two stones.0 -

✭Kill the captain, no drop, log out, log in, quest still in the same state this time but still no credit for killing the captain.
