Activating synergies

This might be a bug, might be something I am doing wrong. When a synergy opportunity comes up and I press the button often my character does nothing but half the animation without activating the synergy. Is this because I am spamming the button?
Sometimes the synergy seems to be overlapped by another. Which does the game choose to show you? Impale or Purify as a preference?
What is the cooldown for activating synergies? For example how long after an impale can I use impale again?
How close do I need to be to the talons to activate impale? Sometimes I am right in the midst of them but get no popup.
Are synergies buggy generally? Or are they just tricky to get the hang of?

✭Are you trying to trigger a synergy you created or a group member created?
All synergy have a % change to trigger (they do not show up ever time)
Then the synergy 'proc' you will get a popup which will say the name of the synergy so you know which you are triggering by the X key.
Even if you get the X key popup, some have a 'secondary' roll to see if you successfully triggered them.1
