I think this is an amazing idea. It would be extremely helpful in difficult dungeon / PvP situations. I think the number of soul gems required to do this would have to be increased greatly from 5 though, especially since veteran players have a large amount of gold paying 500g for 5 empty soul gems is pretty cheap. I would suggest making it cost like 20 soul gems or something. That way it wouldn't be AS abused.
I understand why you would put soul stones in Jewellery but somehow I feel it's better suited for Enchanting. At the most a Jeweler would create the gem then an Enchanter would empower it to accept souls, not seeing a Jeweler involved in gathering soul shards.
Also, not sure about breaking down crafted stones. Not sure how they will be doing it, but if the systems is based on type of gem and quality and possibly size it makes it harder.
For instance, take a gold fine pearled ring, sure you can separate the ring gold from pearl but whatever trait the pearl has it pretty much has.
In Oblivion is was just at special sites at fullmoon at night.I agree with most, however, Black Soul Gems are supposed to be crafted specially.
If memory serves, thru murder, during a full moon, by killing an innocent . I'd love to have that as a crafting method for those babies.
