Random Crashing during game play

ok i keep getting this error randomly with/with-out mods all drivers are up to date and i have tried a lot of "recommended" fixes but to avail i still keep getting the crashing.
also attached is the report generated from the game thank you are any help on this.

    Thank you for posting your Game Consultant Report! I see that your Ping Test and Trace Routing didn't make the connection with our server. That's why you're getting the unknown error. A few things you can try to overcome this issue is:

    -Run a hardline form your computer to the modem.

    -Reconfigure your router to open the following outbound ports to:

    TCP / UDP Ports 24100 through 24131
    TCP / UDP Ports 24500 through 24507

    -Run a DNS Flush on your router:

    -Click on the Start button.
    -Type cmd in the Start Menu.
    -Right-click on Command Prompt and select Run as Administrator.
    -Type "ipconfig /flushdns" and hit enter.
    -If it was successful, you will see the message "Windows IP configuration successfully flushed the DNS Resolver Cache."
    -Power cycle the router to clear the cache so that it doesn't store the bad route.
